Central America is the smallest cluster on this list but the easiest to recommend for a US-based remote worker, same time zones, two-hour flights to half of North America, English workable in any tourist district. Costa Rica's San José runs the most polished nomad ecosystem (Rentista visa, 180-day stamps, mature coworking in Escazú and Santa Ana). Panama City pairs a real downtown with a dollar economy and Panama's Friendly Nations Visa. Antigua and Guatemala City are the highland alternatives; Belize and El Salvador the tropical, raw, cheaper plays.
The friction is uneven safety, Guatemala City and San Salvador have specific neighborhood guidance that changes faster than any blog. Rainy seasons are six months long. Healthcare in tier-1 cities is solid (San José's medical-tourism economy is mature) and limited outside them. Pick the country before you pick the city.
